I turn what I find into a short list of root causes, ranked by impact, and check that list with your leadership and your teams before touching anything.

Along with the team, I design the operating cadences, roles, responsibilities, and what "done" and "ready" actually mean.

I stay hands-on through the first several cycles, adjusting the design against what actually happens rather than what the plan predicted.
